New Patient - Online Registration With The Practice

PreRegistrationIf you wish to register online as a new patient, you are required to complete a registration form and a medical questionnaire. As it can often take sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Click on the two links below to open the forms.

1. Registration Form

2. Online Medical Questionnaire For New Patients

(We ask you to complete a medical questionnaire, because it can take a considerable time for us to receive your medical records).

When you have completed all of the details on BOTH forms click on the "Send" button to mail your forms to us (ALL FORMS MUST BE COMPLETED)

Please Note. If you are newly arrived in the country, you will need to bring to your passport to reception to confirm your date of birth and entitlement to NHS treatment.

Online Access to Patient Services/Emis Access (You will need to register for this service)

Registering online as a new patient does not give you immediate access to our online services. Once you are registered as a new patient, you will then need to register to Online Patient Services/Emis Access

  • book and cancel your appointment with the doctor online
  • order repeat medication online
  • send us your change of details online
  • view your medical records securely online

.Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
